USCIS civil surgeon: Dr. Jose L. Lopez-Gutierrez
Our Civil Surgeon performs an in-person examination, required labs and vaccine administration as indicated by USCIS. Once lab results and vaccines are finalized, we complete and sign your official form-I-693. You will receive the completed signed form in an official sealed envelope. All is done within 48 hours.
Cost: $220. This fee covers the physical exam, completion for form i693, and immigration required lab tests. Any vaccinations are billed separately or can be processed through health insurance.
Hours: Monday–Friday 9:00 AM–5:00 PM.
What to bring to Your Appointment:
Valid government Photo ID: passport, Driver License, or State Identification card. Vaccination Records: Official immunization records (translated into English if necessary). Medical history: list of medications, and documentation of any chronic conditions. Prior Testing Records: Any past Tuberculosis test or chest X-Ray reports
